In a recent landmark independent review of gender identity services for under-18s, a leading clinician highlighted the lack of research on medical interventions in gender care, stating that children were being let down by this gap in knowledge. The review, known as the Cass Review, emphasized the need for gender services for young people to meet the same standards as other NHS care.

The Gender Identity and Development Service (Gids), which was the only NHS gender clinic for children in England and Wales, faced criticism for problems with record-keeping, inconsistencies in clinician’s approaches, and poor leadership. Following the findings of the Cass Review, Gids was closed in 2022.

In response to the closure of Gids, a new service called the NHS Children and Young People’s Gender Service was launched in April 2024. This service aims to provide support for individuals up to their 18th birthday, addressing both the physical and mental health aspects of gender identity.

One of the key issues discussed in the review is the use of puberty blockers and cross-sex hormones for transgender individuals. The NHS has updated its guidelines on prescribing these medications, emphasizing the need for caution and clear clinical rationale when considering such treatments for under-18s.

The debate around transgender rights has also raised questions about access to women-only spaces and the balance of rights between different groups in society. The Equality Act 2010 protects groups with protected characteristics, including gender reassignment and sex, from discrimination. However, there are ongoing discussions about clarifying the law to ensure the protection of women’s sex-based rights.

Overall, the review and subsequent changes in gender identity services highlight the complexities and challenges in providing appropriate care for transgender individuals, while also addressing the broader legal and societal implications of transgender rights.
