A landmark independent review of gender identity services for under-18s has shed light on the challenges faced by children questioning their gender. The review, led by a leading clinician, highlighted the lack of research on medical interventions in gender care, stating that children have been let down by this lack of evidence.

Gender identity is a term used to describe someone’s sense of their gender, which may differ from the sex recorded on their original birth certificate. Transgender individuals may identify as a gender different from the one assigned to them at birth. Some may choose to undergo hormone therapy or surgery to align their physical characteristics with their gender identity.

The closure of the Gender Identity and Development Service (Gids), the only NHS gender clinic for children in England and Wales, was announced in 2022 following concerns about record-keeping and poor leadership. In its place, a new service called the NHS Children and Young People’s Gender Service has been established to provide support for individuals up to their 18th birthday.

The issue of puberty blockers and cross-sex hormones for transgender youth has also been a point of contention. NHS England has implemented new guidelines for prescribing these medications, emphasizing the need for caution and clear clinical rationale. The long-term effects of these treatments, including potential side effects and infertility, have been highlighted in the review.

The debate over transgender rights and their implications for women-only spaces, such as toilets, domestic abuse refuges, and prisons, continues to be a contentious issue. The Equality Act 2010 protects groups with protected characteristics, including gender reassignment and sex, from discrimination. However, there are concerns that women’s sex-based rights may be under threat, leading to calls for further consideration of the definition of “sex” in the law.

Overall, the evolving landscape of transgender rights and the challenges faced by transgender individuals, especially young people, highlight the need for ongoing research, dialogue, and policy development to ensure the protection and well-being of all individuals in society.
